Medical Nutrition Therapy

At Rising Nutrition, we offer a range of personalized nutrition counseling options to empower you on your wellness journey. In one-on-one sessions you’ll work collaboratively with Sarah to develop an in-depth approach to improve your health.

I primarily work with people seeking help with weight loss and any co-occuring needs and/or conditions, not limited to but including:

  • Weight management
  • Prevention and treatment of type two diabetes
  • Insulin resistance
  • PCOS
  • High cholesterol

If you are utilizing in-network insurance to pay for services, the allowed rate is determined by your insurance company. After processing the claim, they will send both you and us an “Explanation of Benefits” which tells us how much (if any) of the fee is your responsibility.

If paying at the time of service and not using insurance, our rates are: 

  • Initial Nutrition Appointment, up to 90 minutes:   $300
  • Follow-Up Nutrition Counseling, up to 60 minutes:   $150

Discounted rates are available in the case of financial need. Contact for questionnaire.

Bariatric Surgery Prep

If you are on your bariatric surgery journey, Rising Nutrition can support you both pre- and post-op.

Rising Nutrition proudly accepts insurance for individual and group nutrition counseling appointments. Learn more about how this works:

Since payment for services is ultimately your responsibility, you should verify your benefits for nutrition counseling prior to our appointment. Please call the number on the back of your insurance card and asking them following questions.

  • Does my plan cover nutrition counseling, procedure codes 97802, 97803 and 99404?
  • Is nutrition counseling covered as a preventive benefit? If so, are there certain criteria that must be met to qualify (e.g. “obesity” or “cardiovascular risk factors”)?
  • Is nutrition counseling covered as a medical benefit? If so, are there any excluded diagnoses? Do I have a deductible to meet before insurance will pay? Will I have a copay/ coinsurance?
  • Do I need a referral or prior-authorization?
  • Is nutrition counseling covered when via telehealth?
  • How many visits are allowed?

Insurance plans accepted include Aetna, Anthem, Blue Cross Blue Shield, Blue Care Network, Blue Cross, Blue Shield, Carefirst, Cigna, Empire, Golden Rule, Highmark, Horizon, Independence Blue Cross, Optum, Oxford, United Healthcare, United Medical Resources (UMR)

Based on licensure restrictions, Sarah is currently able to see patients in the following states: AZ, CA, CO, CT, HI, ID, IL, IN, MA, MI, NH, NJ, NY, OK, OR, PA, TX, UT, VT, VA, WA, WV, WI, WY

Policies

No Show & Late Cancellation (<24 hours notice): 

Our no show or late cancellation fee is $60. For late cancellations only, we will waive the fee if we are able to reschedule you within two weeks and you complete that appointment.

Payment:

A valid credit/debit card on file is required and will be used for self-pay fees, amounts insurance tells us are your responsibility (i.e. copays, deductibles, and non-covered services), and in case of late cancel or no show. You may also keep a HSA/FSA card on file for eligible transactions. Any refunds issued will be less the card processing fees we have incurred.
